We are committed to providing medical care that is affordable for you. Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered. Payment and/or insurance co-pays are due when services are rendered. In order to expedite the check-out process, we ask that co-pays are paid at the time of checking in.

We offer a variety of payment options to meet your needs. Our office accepts payments by cash, checks, and Visa and MasterCard credit cards.

If special arrangements are needed, please speak with our office manager prior to receiving service.

This flexible payment option allows you to pay for your treatment over a period of time so you can focus on what matters most – staying healthy!

Insurance Policy

Our office staff will be glad to assist you with your insurance coverage. When you first visit our office, please bring your current insurance card with you. If you change insurance companies or join another employer’s plan, please be sure to let our insurance department know. If an insurance referral is necessary, please bring the completed form with you to your appointment.

We will fully attempt to help you receive full insurance benefits, and we will even submit insurance claims for you. We are “in-network” with Blue Cross Blue Shield, United Healthcare, Aetna, Cigna, TRICARE, Florida Hospital Healthcare System (FHHS) – Beechstreet network and the PHCS network.

Payment of your bill is ultimately the responsibility of the patient, and timely payment is imperative in keeping the costs under control. Remember your insurance company is seldom motivated to keep the doctors happy but they are highly motivated to keep their members happy. This is a contract between you and your insurance company. You will be responsible to pay deductibles and co-insurance at the time of service. Any additional balances remaining after your insurance company pays is your responsibility.

It is the patient’s responsibility to notify our insurance department of any and all changes regarding insurance filing. If at any time you fail to do so, the patient will be responsible for all charges incurred and subsequently denied by insurance.

Medicare

We accept Medicare assignment, and we will file Medicare claims for you. We also will file your secondary insurance. You are responsible for deductibles and co-insurance at the time of service.

HMO, PPO and any other managed care insurance

If we are contracted with your plan, insurance claims will be filed. You will be responsible for deductibles and co-insurance at the time of service.

Referrals

Due to limitations and time constraints placed on primary care physicians, we cannot process routine referrals in less than three business days. The insurance plans require at least three days to respond to our routine requests. As always, emergencies will be handled as soon as possible.
